They will give an overview of the role of The Health Equity Reform Organization (HERO), progress on the establishment of regional Social Care Networks (SCNs) and provide information on how community provider organizations can contract with SCNs to provide health related social needs services and improve access to mental health services and supports, including the use of peer supporters.
