10:45am-11:45am Breakout/brainstorming sessions
11:45am-12:30pm Report outs from breakout sessions.

Questions to think about for the brainstorming sessions:

  1. For people who are hoping to make a major life transition (for example: moving from incarceration or long-term inpatient stay back to the community, OR starting a job for the first time in years, OR agreeing to engage in substance use treatment etc.) what are the factors that most contribute to the success or failure of that transition?

  1. What important aspects of a person's life/health do you think are most overlooked by clinicians, care managers, service providers or program administrators?

  1. Many people say that they feel isolated, and we know that isolation has a huge impact on people's health/lives. What can we do to better connect people to their communities/social circles?

  1. People are finally beginning to understand the value of peers in many areas of support but in other areas, peers aren't being used. What programs/situations don't usually involve peers but could really benefit from peer support?

  1. What can clinicians/care managers do to create a more trusting relationship with their patients?

  1. What about peer support brings the most value to the support team?

  1. Think about times you were successful at supporting someone's health, independence, autonomy, or recovery. What is the most important thing you did, or the most important aspect of your relationship, that contributed to that success?
