The recently enacted Affordable Care Act (ACA) allows states to elect an option to provide health homes for enrollees with chronic health conditions. The health home provision authorized by the ACA provides an opportunity to build person-centered health homes specifically for people with psychiatric and related disabilities, to achieve improved outcomes and reduced costs. This workshop will provide an overview of the federal standards and requirements for health home services as well as offer recommendations on how to ensure a whole person, recovery centered approach to delivering health home services to people with ongoing health and behavioral health needs.

Self-Directed Care is an innovative approach that supports people using a portion of funds normally given to providers to purchase services and items they identify as critical to achieve their recovery and wellness goals. Most recently, state managed care pilots and federal Medicaid reforms are increasing the opportunities to launch self directed care initiatives. The presenters will provide specific examples of how such programs work, based on their experience of operating some of the most creative and promising self-directed care programs in the country.

The New York State Health Foundation, in collaboration with the New York State Office of Mental Health and the New York State Office of Alcoholism and Substance Abuse, created the New York State Center for Excellence in Integrated Care (CEIC) to support the implementation of evidence-based, recovery-oriented, person-centered and culturally competent services for people with co-occurring mental health and substance use conditions. The Center for Practice Innovations (CPI), in collaboration with the CEIC, offers award winning distance learning technology for practitioners to become proficient in Integrated Dual Disorders Treatment (IDDT). This workshop will provide an overview of these initiatives and how your organization can participate.

In 2009, New York's Care Monitoring Initiative was borne out of an innovative collaboration between New York City and New York State mental health agencies to identify and improve care afforded to 'at risk' individuals with psychiatric disabilities. The presenters will review impressive findings from the Project's efforts in Brooklyn and more recently in the Bronx and will describe how a new Recovery Center initiative will greatly expand the availability of peer support to some of our most vulnerable community members.

Evidence accumulating over the last two decades offers significant challenges to long held beliefs and prescribing practices in psychiatry. Care experts and budget policy makers have converged on a path to identify the appropriate use and not overuse of psychiatric medications. The presentation will feature an update on PSYCKES, an award-winning portfolio of web-based tools designed to support improved clinical decision-making by Medicaid prescribers. It will also feature a challenging counterpoint perspective from a widely read author whose research examines harmful unintended consequences of such medications and cites European research showing high outcomes without prolonged or any medication use.
