NYAPRS Health Homes Update 

August 10, 2011

 

We've learned the following from yesterday's NYS Department of Health
Homes Advisory Group meeting: 

*	Start Date: DOH is considering an adjustment to the timeframe
that would give the field a bit more time to start the Health Homes and
to educate stakeholders accordingly, without slowing the current dynamic
momentum to integrate and improve care. Details are expected by Friday. 
*	DOH staff are also working on the following:     

	1.	A table of rate ranges.  
	2.	Geographic specific (CRG) population data that will help
applicants to describe their capacity to serve the specific kinds of
people that are prevalent in their areas
	3.	A unified multi provider patient consent form in
development that integrates with RHIOs

*         Mergers: DOH will be looking to identify and encourage
'marriages' and mergers of regional health home proposals they believe
work best together. 

*	Health Home Funding: is only going to the one single Health Home
applicant organization; most network services will be leveraged/draw
from existing funding streams, e.g. Medicaid health/bh care, state aid,
housing etc. 
*	Local governments that operate services can apply but have to
have a Medicaid enrolled entity that can accept payment
*	Health home subcontract template: applicants should develop
their own subcontracts; DOH is not providing a template. NYAPRS is
hoping/expecting that progressive Health Home applicants will
subcontract to provide peer bridger/wellness services. 
*	Housing: DOH will be "thrilled" to see housing providers in HH
applicant networks; they will be adding a list of housing providers to
Health Home website to encourage applicants to reach out to housing
providers. NYAPRS has asked DOH to do the same with peer providers.  
*	Case Management: 

	1.	DOH affirmed that they're expecting to auto assign HH
clients into Health Home networks that include their existing case
management program; they're not "looking to move existing stable
patients" out of relationships and service arrangements that are
working. 
	2.	OMH is looking at how their case management non-Medicaid
wrap around dollars can best be deployed in this environment, and will
be exploring this with DOH. 
	3.	DOH confirmed that more slots beyond current OMH
targeted case management, HIV/AIDS COBRA and OASAS MATS levels will be
needed to serve the magnitude of individuals to be served via HHs. 

*	HH priority groups at program roll out will be groups who are
"least engaged in medical, mental health and substance use treatment
services and those who have high acuity scores." 
*	Choice: People will be initially free to opt out although DOH
hopes that HH networks will work hard to engage, satisfy and retain
assignees. They are seeking a waiver that might make the HH assignment a
mandatory one. 
*	Beneficiary Education: DOH acknowledges this needs more
attention and is eager to work with groups like Medicaid Matters, NYAPRS
and others to raise beneficiary awareness and readiness. 
*	Phase 2: DOH expects to award most Health Home applications in
the coming weeks/months; a phase 2 might come later if/as needed. 
*	Outreach and Engagement:   DOH is seeking guidance from CMS if
they can get approved reimbursement for outreach and engagement. It's
not included in the list of core services in the ACA law but CMS seems
open to exploring it. 
*	Standards for Evaluation/Performance Measures have not been
approved yet by CMS. 
*	Dual Medicaid/Medicare enrollees: DOH is reimbursing for them in
this initiative. 

*         HH Regulations: DOH is focused on producing outcome measures
rather than regulations.

The pace of change continues to accelerate. Although DOH has just
announced that it is considering an adjustment to the current timeframe
that would give us all a bit more time to start the Health Homes, there
is still a need to move fast and be prepared! 

 

All providers are looking closely at what services they can provide, how
they can provide them in the most effective and efficient manner and how
best to market them. Some providers are actively seeking to be part of
HH networks while others are looking at the same issues and seeing if
they are better positioned to apply for Health Home status themselves.
