POSITION DESCRIPTIONS

 

PROS PROGRAM COORDINATOR

The PROS Program Coordinator is responsible for maintaining the day to
day supervision of assigned staff members in the PROS Program at HALI U
West.  These job duties must be performed in a manner which is in
keeping with the mission statement of Hands Across Long Island, Inc. and
must also project a positive image of the employees, individuals, and
agency of Hands Across Long Island, Inc. to the public, participants,
other agencies, families and Board of Directors.  The PROS Program
Coordinator reports to the Deputy Director.

The essential functions of this position include, but are not limited
to:

- Train staff on psychiatric rehabilitation model including person
centered planning

- Identify & develop services geared to meet needs of growing and
changing population.

- Review and sign-off on individual's case records

- Be available to individuals in the PROS program

- Ensure billable activities reflective of individual's goals

- Follow Best Practices guidelines and ensure continuing quality
improvement

- Provide support and weekly supervision to all assigned staff members

- Attend scheduled meetings with Deputy Director

- Manage after-hours assignments

- Job responsibility requires working both on site and off site

- Manage staffing pattern to meet regulations

- Identify training needs of staff

- Assist in creation of cultural competence in all services provided by
agency

- Participate in weekly team meetings

- Prepare written monthly report for the Deputy Director

-Provide timely documentation to supervisor and HR of qualifications
necessary to provide services

- Promoting empowerment, recovery and team approach

- Attend County & State meetings and conferences

- Plan training & education events, and provide training and education

- Give presentations on behalf of HALI

- Prepare employee performance evaluations

- Maintain confidentiality

- Oversee maintenance of records for regular audits

- Maintain orderliness of work environment

- Additional duties as assigned  

QUALIFICATIONS: Must be a Licensed Practitioner of the Healing Arts with
a minimum of 3 years supervisory experience and experience in providing
clinical supervision a must.  Must have experience with individuals with
co-occurring disorders, mental health and substance abuse and be a team
player. Willingness to learn, must be computer literate, excellent
written and verbal communication skills, high-energy person and have
excellent organizational skills. Personal experience with mental health
system a plus.

REHABILITATION PRACTITIONER- EMPLOYMENT SPECIALIST

The Rehabilitation Practitioner- Employment Specialist will assist
people living with mental health condition in obtaining self-directed
goals of employment. These job duties must be performed in a manner that
is in keeping with the mission statement of Hands Across Long Island,
Inc. and must also project a positive image of the employees,
participants, and agency of Hands Across Long Island, Inc. to the
public, individuals, other agencies, families and Board of Directors.
The Rehabilitation Practitioner- Employment Specialist reports to the
PROS Program Coordinator.   

 The essential functions of this position include, but are not limited
to:  

*	Provide quality psychiatric rehabilitation interventions to
individuals and collaterals  in a timely and responsive manner
*	Provides employment assessments to participants

*	Engages participants into employment services
*	Assist the participant in job placement and unlimited
follow-along services after the individual is employed

*	Must relate in a positive manner with employers in the
community.
*	Assist the individual in understanding the impact of their
mental health conditions, treatment and medication on vocational
functioning.
*	Advocate effectively for participants with employers and others.
*	Provide education and support to the employer which may include
job accommodations and follow-up contact with the employer.
*	Initial and on-going job development/placement in the community.
Makes at least 6 face-to-face employer contacts per week on behalf of
participants looking for work
*	Provides "job coaching"  within the context of the PROS program
*	Maintains a caseload, provides all documentation required for
said caseload in a timely and accurate manner in accordance with
regulations
*	Performs all duties associated with the HALI Employment Network
*	Has a working knowledge of entitlements, benefits and work
incentive programs
*	Works collaboratively with VESID
*	Collects and enters employment data into the NYISER database
*	Maintains the HALI U Employment Center
*	Organizes and provides informational events for participants and
employers
*	Promotes individualized, competitive employment
*	Other duties as assigned

QUALIFICATIONS:

Minimum 3 years experience as employment rehabilitation practitioner,
experience in working within a team planning environment. Have a general
knowledge and experience in job development, job marketing and job
securing. Possess a broad working knowledge of occupations and jobs and
be able to identify a participant's interests, strengths, skills,
abilities and challenges and match these qualities with jobs.
Willingness to learn, excellent written and verbal communication skills,
high-energy person, personal experience with mental health system a
plus.  Computer skills a must.

REHABILITATION FACILITATOR

To run various group and activities for the PROS Program. These job
duties must be performed in a manner in keeping with the mission
statement of Hands Across Long Island, Inc.  The Rehabilitation
Facilitator must also project a positive image of the employees,
participants and agency of Hands Across Long Island, Inc. to the public,
individuals, other agencies, families and Board of Directors.  The
Rehabilitation Facilitator reports to the PROS Program Coordinator.

 The essential functions of this position include, but are not limited
to:

-  Provide quality psychiatric rehabilitation interventions to
individuals and collaterals  in a timely and responsive manner

- Collaborate with individuals to identify goals & objectives

- Collaborate on services to meet those goals & objectives and discuss
progress

- Develop an individualized recovery plan (IRP) with the individual.
Review and update as appropriate

- Document information in each record in a clear & timely manner

- Maintain case load 

- Maintain all paperwork in a timely fashion

- Participate in weekly supervision with Program Coordinator

- Participate in weekly meeting with PROS team.

- Participate in monthly staff meeting

- Participate in trainings

- Maintain cleanliness of work area

- Other duties as assigned

QUALIFICATIONS:

Experience in working within a team planning environment. Willingness to
learn, excellent written & verbal communication skills, high energy
person. Experience with group facilitation and in the mental health
field. Must be a team player.  Must have the ability to perform all
functions with or without reasonable accommodations, personal experience
in mental health system a plus.  Must be computer literate.

REHABILITATION PRACTITIONER- CASAC

The Rehabilitation Practitioner- CASAC will provide individual and group
sessions to individuals, perform evaluations, crisis intervention, and
treatment planning regarding all areas of co-occurring (mental health &
substance abuse) disorders.  These job duties must be performed in a
manner which is in keeping with the mission statement of Hands Across
Long Island, Inc. and must also project a positive image of the
employees, individuals, and agency of Hands Across Long Island, Inc. to
the public, individuals, other agencies, families and Board of
Directors.  The Rehabilitation Practitioner- CASAC reports to the PROS
Program Coordinator.

The essential functions of this position include, but are not limited
to:

- Provide quality psychiatric rehabilitation interventions to
individuals and collaterals in a timely and responsive manner

- Collaborate with individuals to identify goals & objectives

- Collaborate on services to meet those goals & objectives and discuss
progress

- Develop an individualized recovery plan (IRP) with the individual.
Review and update as appropriate

- Document information in each record in a clear & timely manner

- Maintain a caseload

- Facilitate classes as requested. Includes planning, taking attendance,
evaluation of group and completing progress notes

- Conduct individual counseling sessions

- Complete all documentation in a timely manner in accordance with state
and agency regulations

- Develop and modify curricula so as to meet needs of individuals for
education and therapeutic groups

QUALIFICATIONS:  CASAC or CASAC-T, minimum 1 year experience in
co-occurring disorders, comprehensive understanding of mental health and
substance abuse co-occurring disorders, knowledge of counseling
techniques- including treatment planning, knowledge of group process,
strength-based principles of treatment, excellent interpersonal skills,
effective written and verbal communication skills, crisis intervention
skills. Personal experience with mental health system a plus.  Must be
computer literate.

- Assist in clarifying the benefits of psychiatric rehabilitation
services to interested individuals & parties 
- Demonstrate cultural competency  
- Actively participate in ongoing training as needed to meet all
certification standards and credentialing policies 
- Provide timely documentation to supervisor and HR of qualifications
necessary to provide psychiatric rehabilitation services 
- Collect QA data and participate in the development of continuous
quality improvement strategies based on relevant information and
accrediting standards.

- Other duties as assigned

QUALIFICATIONS: Licensed Practitioner of the Healing Arts, minimum two
years experience working with individuals with severe and persistent
mental health conditions, and experience in working within a team
planning environment. Willingness to learn, excellent written and verbal
communication skills and high-energy person. Personal experience with
mental health system a plus.  Must be computer literate.

PROGRAM CLERK

The tasks of the Program Clerk will include typing, filing, faxing and
handling other specific matters as the responsibility of the individual
increases.  Schedule is flexible.  These job duties will be performed in
a manner which is consistent with the mission statement of Hands Across
Long Island, Inc.  The Program Clerk must also project a positive image
of the employees, participants and agency of Hands Across Long Island,
Inc. to the public, Individuals, other agencies, families and Board of
Directors.  The Program Clerk reports to the Program Coordinator.

The essential functions of the position include, but are not limited to:

*	Filing papers
*	Responding to phone messages as needed
*	Answering phones
*	Photocopying materials and various documents
*	Organizing office systems including files and records
*	Entering data into online documentation system (AWARDS)
*	Assist individuals in completing initial screening paperwork and
follow-up paperwork
*	Follow up calls to individuals as needed
*	Maintain cleanliness of work area
*	Maintain confidentiality
*	Other duties as assigned

QUALIFICATIONS:

The minimum qualifications for this position are: ability to organize
and maintain files and office procedural systems, excellent
interpersonal skills, effective written and verbal communication skills.
Must be computer literate. Personal experience with the mental health
